At Jewel City Treatment Center, we assure confidentiality of the client and the client’s information in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and Part 2 of Title 42 of the Code of Federal Regulations.
Jewel City Treatment Center will only use and disclose protected health information (PHI) if either (1) the HIPAA privacy rule specifically permits or requires it; or (2) the individual who is the subject of the information gives authorization in writing.
Jewel City Treatment Center recognizes the confidentiality of client record information and provides safeguards against loss, destruction, or unauthorized use. This Written procedure governs the use and removal of records and the conditions for release of information.
- Client records, or any part of the record, must not be removed from the Jewel City Treatment Center facility premises by any staff member or employee except as required by law and approved by the Clinical Director or Program Director.
- Client identifiable information must not be disclosed or released without the written consent of the client or to any person not directly concerned with the care of the client, except when disclosure is authorized by law.
- Client records may be reproduced only with the approval of the Program and/or Clinical Director.
- Client records may be inspected, copied, changed, or removed from their protected health information only with the written request of the client.
