What is Discharge Planning?
Discharge planning is a crucial part of recovery, and includes planning for your time once you have completed your addiction treatment program. It’s important to plan ahead for your life outside of rehab by prioritizing your physical, mental, and emotional health and well-being.
At Jewel City Treatment Center, we work with you to create a discharge plan that takes the form of a living document. This plan includes recommendations and plans for how you’ll proceed once you leave substance abuse treatment, and is an important aspect of long-term recovery.
What to Expect: Our Discharge Planning Process
Upon completing an addiction treatment program at JCTC, you’ll be equipped with the tools to enter back into a world with more responsibilities, increased flexibility, and less accountability. That can be exciting since many opportunities and freedoms are available to you outside of a rehab facility. However, it can also be stressful, anxiety-ridden, and scary, which can lead you to relapse. To help you stay healthy and sober, we’re here to take some of the stress and anxiety out of the transition process so that you can live the life you want.
Your discharge plan should be individualized and tailored to your needs and lifestyle and will likely include:
– Random drug or alcohol testing
– Individual, group, and family therapy
– 12-step meetings
– Wellness activities, like yoga and meditation
– And more
Remember, the goal is to prioritize your health so that you can prevent relapse and maintain your sobriety in the long run, so your plan should be realistic and achievable.

We will work directly with you to craft a discharge plan you can follow and share with a trusted friend or family member, who understands your needs and goals, and provides accountability.

However it is important to note that your discharge plan will be exactly that: yours. It will incorporate evidence-based components catered to your unique needs and goals for treatment, like follow-up appointments, support groups, and therapy sessions, which have the highest likelihood of making your discharge a successful one.
